Complete the form if you would like your event to be attended by a civic head.
Please make sure that you submit your request no later than three weeks prior to your event. Your request will be considered within 10 working days.
If you have a risk assessment relating to this event, you will have an option to upload it at the end of the form.
Once you have started, you must continue to the end and submit the form.
Please note that you only have 30 minutes to fill out each page and you will not be able to come back to the form at a later time because your answers will not be saved.
