You must live, work or represent a body that is in the vicinity of the premises in order to be able to make a statement.
All comments must relate to the four licensing objectives, which are:
- to prevent crime and disorder
- public safety
- to prevent public nuisance
- to protect children from harm
Please note that you only have 30 minutes to complete each page. Your answers will not be saved until you submit the form, so once you have started you must continue to the end. You cannot leave the form and come back to it later.
