Mediation / conflict resolution request form

Please note that mediation can only be arranged if both parties agree to undertake it.  Mediation does not necessarily mean both parties being in the same room at the same time.

The submission of this form is taken as confirmation that you agree to undertake mediation and share the information submitted with Catch 22 mediation service. 

Where both parties do not agree to mediation Catch 22 may be able to provide a service to help manage conflict resolution. 

We will let you know either way if we have managed to obtain agreement from your neighbour for mediation to take place. 

Please note: when you start the form you must continue to the end and submit it. You have 30 minutes to fill out each page and you will not be able to save the form and come back later.
