This is a non means tested grant of up to £15,000 to allow you to be safely discharged and prevent longer stays in hospital. Works can vary depending on the circumstances of the individual. But may include installing stair lifts, level access shower rooms and ramps.
To be eligible for the hospital discharge grant, you must need adaptions in your home to be discharged from hospital, and you must be referred by a health professional.
