You are about to make a claim for housing benefit and/or an application for council tax reduction. Council tax reduction is the scheme that replaced council tax benefit. This form should not be used to apply for council tax discounts or exemptions.
Please note that this form is not to be used as an application for single person discount for a council tax account.
You must answer all of the required questions (marked REQUIRED) in each section of the form before you will be able to move forward to the next section.
The form should take approximately 20 minutes to complete, longer depending on your circumstances. Once you have started, you must continue to the end and submit the form. You only have 30 minutes to fill out each page and you will not be able to save the form and come back later.
You will need to provide information to support some of the answers you have given in the form. You may wish to get items relating to your tenancy, income, bank accounts, ID ready to support your application. Original documents can be posted to Norwich City Council, City Hall, St Peters Street, Norwich NR2 1NH.
The person completing the form must be the claimant/applicant and you will be asked to declare this before submitting the form.
You can only assume that your claim/application has been successfully submitted upon receipt of a confirmation email.
